FEI DELEGATE PRE-TRAVEL ACTION CHECKLIST | ||
Note: the checklist below summarises the actions that must be completed before you fly to Japan and reflects the requirements imposed by the government of Japan. Failure to comply may result in being denied boarding your plane or entering in Japan. Please follow in detail all the instructions provided below and in the documents referenced. Contact us if unsure. |

If you need to bring medication for personal use into Japan: comply with Japan’s law. |
||
Monitor your health for 14 days before arrival in Japan, including: - Taking your temperature daily - Proactively monitoring for any Covid-19 symptoms |

Get a smartphone with iOS 13.5 and Android 6.0 version and above. |
||
Download the OCHA and COCOA apps on the smartphone (NB: COCOA will not be used until your arrival in Japan, but you must download it before travelling). |
||
Register in OCHA and complete the “Before You Travel” process by entering all required data (including your health information). Refer to Annex I in this document for details. |
||
Complete your pre-travel tests exactly as explained in our Communication II published on the Covid Liaison Officer Paralympics pages of the FEI’s website. Make sure that the tests are taken within the required time window (contact us if in doubt). If using self-testing Meenta kits, follow the instructions carefully and check this video before taking your test. If not using Meenta self-testing kits make sure the test result certificate is based on the template designed by Japan’s government. If the template is not used, the certificate MUST include all the information listed in Communication II. Please note that Tokyo 2020 has informed us that a number of people in Olympic delegations have been denied plane boarding/entry in Japan for failing to meet this requirement.
In case you need to use a different provider than MEENTA to take the Covid-19 Tests, or if you need to show your airline that Meenta are an approved testing provider, find below the lists of approved Testing Providers approved by the Japanese Government: - LIST 1; - LIST 2 (MEENTA is found in line 2429). |

Upload in OCHA your negative Covid-19 test result taken within 72 hours of departure to Japan. |
||
Check that OCHA has generated a Quarantine QR code (this happens when all required data as mentioned above has been entered). You will show the Quarantine QR code to the airline staff when boarding the plane, and to the airport officials during the arrival process once in Japan (see section 2 below for details). |
||
Take screen shots of the Quarantine QR code and the negative 72 hours Covid-19 test result, and keep them in your phone so you have them readily available once at the airport in Japan in case the OCHA app stalls. These documents must be presented at every step of the arrival process (see section 2 below for details). |
||
Print out copies of: - Any Covid-19 relevant medical information (for example and as applicable: vaccination status; prior positive Covid-19 test results - including antibody tests, medical records related to any condition that increases your risk of a severe Covid-19 infection). - All your pre-travel Covid-19 tests results. - (if applicable) any permit to bring personal medication into Japan. |

Pack in your hand luggage, ready to show at the airport: - Passport - Pre-Valid Card (PVC (PIAC)) - Printed copy of the negative COVID-19 test result certificate from a test taken within 72 hours of departure. - All COVID-19 relevant medical information as mentioned above. |

Keep your smartphone battery charged during the flight so it can be used on arrival. |

Install and register with the Japanese government’s Online Check-in and Health report App OCHA (using the ICON password). |

Install the Contact Confirming Application (COCOA) app on your smartphone. It will not function until you are on the ground in Japan (will need to be activated after landing). |

The FEI will complete and submit your 14-day Activity Plan, in line with the list of permitted destinations. |
||
14 DAYS BEFORE YOU TRAVEL | ||
Proactively monitor your health and report health status in OCHA when your Activity Plan is approved. |

If you experience any symptoms of COVID-19 in the 14 days before departure, do not travel to Japan, consult with a medical professional and inform your COVID-19 Liaison Officer immediately. |

WITHIN 7 DAYS TO 96 HOURS OF YOUR DEPARTURE | ||
In order to be granted entry in Japan, all Tokyo 2020 Paralympic Games must complete pre-travel tests, as outlined in the table below. For persons who will have stayed in the countries identified as “high-risk” (Delta variant) by the government of Japan during the 14 days preceding their arrival, specific testing schedules apply. |

ALL EXCEPT GROUPS 1&2 Two Covid-19 tests, on two separate days, within/less than 96 hours of the departure of your flight to Japan (first international flight if you are on an indirect route). At least one of the two tests must be taken within/less than 72 hours of departure. There must be full 24 hours between the 2 tests. |

GROUP 1 - Indonesia, Kyrgyzstan (updated 12 August) Daily testing for 7 days before departure to Japan. |
The placement of countries within the two groups and the measures that must be followed, is based on various factors related to COVID-19 in each such country, which are determined by the Japanese Government. These groups are subject to change. You will need to have your tests results available to show when boarding the plane and on arrival in Japan. For your pre-travel tests please follow in detail the instructions provided in FEI Communication II of 21 July. |

GROUP 2 - Afghanistan, Bangladesh, India, Maldives, Myanmar, Nepal, Sri Lanka, United Arab Emirates and Zambia (Updated 12 August) Daily testing for 3 days before departure to Japan. |
